"If I saw Dr. Carly at another practice, do I need to schedule a new patient visit?""Yes! I need you to establish care at this practice. This means you will need to fill out a new patient intake form and sign new consent and procedural forms. It is a good time for you to review your own health history, re-examine your health goals and update me with any new diagnoses, family history and/or treatments you have had. However, because I recognize this takes additional time, I am offering a 25% discount on All New Patient Visit fees for anyone that schedules a New Patient Visit for the remainder of 2019.

"Why are your appointment hours limited during the week?""One of the principles of Naturopathic Medicine is “Docere”, or Doctor as teacher. Outside of patient care, I teach anatomy and physiology at Alexandria Technical and Community College. I love being able to teach future healthcare providers. I also serve on the Alexandria Dollars for Scholars Board, City of Alexandria's Cultural Inclusiveness Committee, and Douglas Country Relay for Life Committee. Most importantly, I am a mom of three young kiddos who fill up my heart and time and I attempt to provide some sort of real-world example of work-life balance for my patients.

"Do you carry supplements onsite at your practice?""No, I do not carry supplements onsite, however, you can purchase supplements through multiple online dispensaries I offer. For the inconvenience I always offer an additional 10% off your supplement orders. Additionally, the majority of the dispensaries offer free shipping for orders over $50.

"Do you bill insurance?""No, Evergreen Natural Family Medicine does not participate in insurance plans. This means that your visit fees are due at the time of service. I can provide you with a superbill that has the insurance codes for your visit. You can submit this superbill to your insurance for potential reimbursement and application to your out-of-network deductible and out-of-pocket costs. I do, however, accept payment with Health Savings Accounts (HSA) and Flex-Accounts.

“Do you do telemedicine?”Yes, Evergreen Natural Family Medicine is a fully virtual practice. Phone visits (and video visits if needed) are available for Minnesota and North Dakota residents only.

“Can you be my primary care provider?”No, while I am trained as a primary care provider, I am not offering primary care services for a number of reasons. First, I have limited hours and do not provide “on-call” services to my patients. Second, Minnesota limits Naturopathic Doctors’ scope of practice which means I am not able to administer injections or provide prescriptions for pharmaceuticals, and while these aren’t treatments that I would always recommend, accidents and illnesses do happen that require these modalities and I like you to be established with a provider to help expedite your medical care in these times of need. Lastly, we live in a community filled with many amazing medical providers that I am happy to partner with as part of your health care team. Collaboration is possible and encouraged for your success.
