All visits with Dr. Carly will take place as phone visits (or video conference as needed) and are available for *Minnesota and North Dakota Residents only* as she has transitioned her practice virtual.  Dr. Carly does not provide primary care services at her practice.  If you have not yet seen Dr. Carly at Evergreen Natural Family Medicine, you will need to schedule a new patient visit, even if you have seen her at a previous practice.  All subsequent visits should be scheduled for a follow-up visit, unless told specifically to schedule a "brief" follow-up phone visit.  A credit card is requested to hold your appointment and will not be charged until your appointment or per our cancelation policy.  You will receive an email after Dr. Carly confirms your appointment that will provide you with any additional forms or questionnaires that need to be completed on your Charm Patient Portal prior to your appointment.  These questionnaires also go over Evergreen Natural Family Medicine's billing and cancelation policies.
